In OLDCARTS, 'A' stands for 'Aggravating and Alleviating Factors.' This is a two-part assessment that explores what makes the pain worse and what makes it better. Questions include: 'What activities, movements, or positions increase the pain?' (e.g., walking, deep breathing) and 'What have you found that relieves the pain?' (e.g., rest, a specific medication, heat, or ice). This information is extremely valuable. It helps identify the source of the pain, guides recommendations for activity modification, and evaluates the effectiveness of current or past treatments, forming a basis for the nursing care plan.
Break down complex topics into smaller, manageable chunks for easier recall.
