Advancing age is the single most important and strongly associated demographic risk factor for the development of Benign Prostatic Hyperplasia (BPH). The prevalence of BPH increases progressively with age. It is rare in men under 40, but histological evidence of BPH can be found in about 50% of men by age 60, and this rises to over 90% by age 85. This age-related increase is closely linked to hormonal changes, particularly the gradual rise in the ratio of estrogen to testosterone and the continued intraprostatic conversion of testosterone to the more potent dihydrotestosterone (DHT), which stimulates prostate cell growth. While other factors like family history, race (higher incidence in Black men, lower in Asian men), and lifestyle (obesity, diet) play a role, age is the universal and non-modifiable driver of the condition. This is why BPH is primarily considered a disease of the aging male population.
