Primary VZV infection (varicella) and reactivation (zoster) both present with vesicular rash. Rising IgM titers indicate recent primary infection or reactivation. PCR can detect virus but doesn't distinguish primary vs reactivation. Clinical presentation differs (dermatomal vs generalized).
