Strongyloides stercoralis can be detected in sputum cytology, particularly during the hyperinfection syndrome in immunocompromised patients. The filariform larvae may migrate through the lungs and be coughed up in sputum. They appear as long, slender nematode larvae with short buccal cavities and notched tails. Other parasites occasionally found in sputum include Paragonimus westermani (lung fluke eggs), Echinococcus (hydatid sand), and migrating Ascaris or hookworm larvae. Detection of parasites in sputum is significant as it may indicate disseminated infection requiring specific antiparasitic treatment.
