After total thyroidectomy and often radioactive iodine ablation for differentiated thyroid cancer, thyroglobulin (Tg) should be undetectable in the blood. The presence of detectable or rising serum Tg is the most sensitive indicator of persistent or recurrent disease. Measurement must always be accompanied by testing for anti-thyroglobulin antibodies, which can interfere with the assay.
